Customized Test Requisition Forms

Customized forms are available to all Macula Risk Testing Centers. These simplify the order process one step further. To order Customized Requisition Forms, fill out the Macula Risk Custom TRF Form and return via either fax 1 (866) 964-5184 or e-mail. Ordering Kits

Kits usually take between 5-7 business days to arrive after the order has been received. Re-order forms are included with all kits or can be downloaded here. Fax 1 (866) 964-5184 or E-mail the scanned form to customerservice@macularisk.com

Pre-printed Chart Sticker-templates

Clinics have found these templates useful. This template works with the 3M 3100 line of labels. The eye doctor fills in the follow-up, AREDS vitamin and home PHP plan while the test is being taken with the patient. The technicians will then be able to begin executing these plans as soon as the test results are returned, instead of waiting for the doctor to review the findings and develop a plan.
